First Visit

At your initial appointment we will review your health history and thoroughly examine your tooth in order to present you with the proper course of action. It is important to us that you fully understand your treatment options in order to make an informed decision. We will take the time to listen to your concerns and answer whatever questions you may have.

Please assist us by providing the following information at the time of your consultation:

• Your referral slip and any X-rays if applicable.
• A list of medical conditions that you have (i.e. diabetes, high blood pressure, artificial joints, etc).
• A list of medications you are presently taking.
• If you have dental insurance, please bring your insurance information.

IMPORTANT: A parent or legal guardian must accompany all patients under age 18 at the consultation visit.
